Adaptation to a stable flow of dopamine becomes difficult as the flow is never constant as it is. And seeing how it can't predict the future, it manages to prepare for potential flushes.

In the US medicine is a cash business. Health is pay to win. It doesn't matter how much or how little it works, what matters is will it make a profit. If the answer is no it's not pushed.
